EWS88 MT
Designed more for the professional musician than the consumer market, the EWS88 MT arrived in 1999, and was based on the Asahi Kasei AK4524 audio codec. This was not a card for PC gaming, as it provided no support for any known sound card standards, aside from possibly General MIDI.

The EWS88 MT came with a 5.25" drive bay module that provided eight stereo inputs and outputs via phono plug connectors, as well as two MIDI connectors on the front panel.
The backplate provides a 3.5mm stereo jack for audio monitor output (which has a 16-bit DAC), two more analogue phono sockets, and a female DB-type connector for connection to the breakout box. You can also connect this using the 'IDE'-type socket (IEC?) on the card if mounting internally within your PC.
The main chip on the card is the AK4524 from AKM Semiconductor, and there are four more of these inside the breakout box to handle its various inputs and outputs. This audio codec has its own internal ADC and DAC.
Up to four EWS88 MT cards can be connected in the same system, allowing for up to 40 inputs and outputs (32 analogue and 4 stereo digital).
The format of your recordings is of course entirely up to the software you wish to use with the EWS88 MT.
Bundled software comprised of Emagic's MicroLogic AV 4.0 (a special version designed for the EWS88 MT), Samplitude Basic (a multitrack audio editing suite), WinJey (a versatile player for audio CDs, WAV files, MP3, and MIDI files), and BuzZ (an advanced freeware tracker tool).
